Make Monthly Donations

Becoming a Monthly Giver is easy! Just follow these steps:

1. Send us a copy of one of your bank cheques with the word "Void" written on the face.     
                                                               OR
Authorize the Upper Credit Humane Society to bill your credit card on a monthly basis in the space indicated.

2. Write down the amount you wish to give monthly, to be withdrawn on the 15th of every month.

3. Sign this form in the area indicated.

4. Return to the Upper Credit Humane Society.
